The Anthony Muñoz Foundation is pleased to facilitate your request for an Anthony Muñoz autograph.
For personal memorabilia autograph requests, please complete the Personal Autograph Request Form.
For autograph requests for fundraisers, silent auctions or youth organizations, please complete the Organization Autograph Request Form.
Autograph Request Items
The requestor must provide the item. The AMF limits one (1) item per requestor per calendar year.
For requests related to fundraisers, silent auctions or youth organizations, no monetary donation is required. For all other requests, we ask that you make a donation to the Anthony Muñoz Foundation.
Minimum Donation Requirements
Anthony personalizes each item (i.e., "To John..."). If an item is for an auction or raffle, you will be asked to send us the item to be signed, along with the related payment, once the recipient is known.
Item Shipping and Pick Up
AMF is not responsible for any shipping and packaging charges. If sending an item in to be signed, please be sure to include return postage.
Items may be dropped off and picked up at the AMF office Monday through Friday between 9 a.m. and 5 p.m. Turnaround time on getting an item signed is typically one week. Please call prior to pick up.
Before receiving the signed item, information pertaining to the amount raised must be provided.
Autograph Request Guidelines
Due to the large volume of requests we receive, there are no exceptions to the guidelines. Anthony is extremely generous with his autograph, but submitting a request does not guarantee that you or your organization will receive an autograph.
Upon receipt of all requested materials, an AMF representative will contact you to discuss the status of your request.
All proceeds benefit the Anthony Muñoz Foundation's eight impact programs.
